What series was that? I don't know why, but my assumption is Naruto.

While I guess it's pretty normal to expect the main character not to die (unless the series loves Biblical symbolism), the two of them both do have serious cases of Shonen Protagonist Syndrome.

Wasn't it something like Ichigo never dies or something? It wasn't that Ichigo never dies, although that seems to be the case at times. It's the fact that the plot twists aren't used to enhance the story but instead are used to get out of writer's block. Which involves a lot of shunpo hax and out-of-the-ass last-minute concepts. Naruto doesn't really suffer from this as much as Bleach does, though.

SATAN, THE CHAMPION OF THE WORLD! Funnily enough, Goku was meant to die at the end of the series. This happened when Namek finally exploded, but the fans insisted that there would be more DBZ, so the creator brought him back. To try again, he made Goku sacrifice himself when Cell was about to explode Earth. Same thing happened and we got the Buu saga.
